Patient Information

Your First Visit

Walk into our lobby where you will be greeted by our friendly staff. We will provide you with new patient paperwork that includes a health questionnaire, request for your insurance information and contact information. We will also provide you with information to familiarize you with our office and how we handle things like patient privacy.

Initial Examination

Your initial dental exam will take around one hour. The expert staff at Fortune Dental will ask you questions, get to know your oral hygiene routine, and listen to your health concerns.Our dental hygienist will clean your teeth then the Dentist will complete your examination and make recommendations for proposed dental treatments.

Pain Concerns

We work to ensure our patients feel comfortable, and that includes minimizing their pain as much as possible. If you are undergoing a dental procedure, Doctor will meet with you to discuss your pain tolerance and create a plan for pain management. Working together, we will treat your dental health problems in a way that minimizes pain and enables you to get back to your life faster.

Office Policies

Appointments

Your scheduled appointment time is valuable and has been reserved specifically for you. If it is necessary to reschedule your appointment, we require at least 48 business hours notice. Otherwise, a charge of $75 per hour scheduled will be incurred.

For all I.V. sedation appointments, a minimum of 7 days notice is required or you may be charged a fee of $150 for a missed or short notice canceled an appointment.

We are aware that unforeseen events sometimes require missing an appointment, however, we do ask for this courtesy, not just for us but for our other patients.

Insurance

Fortune Dental will be more than happy to bill your insurance company on your behalf as a courtesy. However, we cannot always know as to what your insurance may or may not do in regards to payment, no matter how much we try. Thus, we are not responsible if your insurance company does not pay for our professional services and materials. You are therefore financially responsible for any services and/or materials that are not covered by your insurance.

Services may be paid with Cash, Debit, Visa, Mastercard, or American Express. In services requiring lab work, (Crowns, Bridges, Retainers, Dentures, and Partials) a minimum of 50% deposit is required and the remaining 50% is due upon delivery of materials.

Payment is required on the date of service unless prior arrangements have been made for payment.
